During the quest "A Tiny Little Errand", neither the Magic Deck, nor the Loaded Dice ever actually appear in your inventory, or at least, I can't seem to see them anywhere. The quest still progresses regardless, there's just no item entry in quest items for them. There might be others like...

At the beginning of the third main story quest (after returning the gauntlet), there's a conversation between new recruits where Wedge is referred to as "Ledge", while his name in dialogues is "Wedge". I assume this was left in from an initial attempt at obfuscating the potential copyright...
