The "total" can under certain circumstances change during the course of the game! So the 5/6 suggests that there's some last "secret" for you to uncover (I'm guessing it has to do with Twi-sight)! The "?" symbol means "chests and secrets", so it's not a quest you're missing!
To sanity check: did you skip the Ghastly Gathering quest, or the Spectral Ball quest? The Spectral Ball quest is the one which gathers over a dozen ghosts to the ball, while the Ghastly Gathering is the one that gives you the key to Captain Bones' cabin.

He needs to open the chest in his own character's world to get the treasure map! The treasure gained from the map is connected to the current world (a fertilizer), therefore he needs to obtain it by opening the chest in his own world.
Oh, I see now! Those are Arcade Mode achievements unlocked by playing that mode!
You can view what achievements belong to Story Mode in-game by looking in your journal
Hey! The Steam achievements only unlock once and are tied to your Steam account, so starting a new file won't trigger them again if you got them while playing multiplayer!
