Hi there.
I've made this happen once or twice in my own games, but I don't normally record. A streamer I follow managed to get it to happen on stream today, so I clipped it. I figured that's the most useful information I can give.

It's probably a desync issue, the host player sees a ghost and can revive him without issue.
