[Gameplay] Dying from poison in co-op at the same time you use a phaseplate glitches the game out?

Player 2 was using the phaseplate to teleport in the poison battle area of Mt. Bloom at the same time they took their final tick of poison damage. As a result they were frozen in place as an orb of light mid-fire, unable to to do anything.

I think the final-poison-tick had something to do with it, at least.
