You know when you have the item because you get the "received item" noise (from fishing, opening presents and a few others) and it comes up in your (what I like to call) "item killfeed" in the bottom left. You're safe to shield once you've heard the noise and you present the item above your head. You really need to get the frame timing on that one to be able to get 4 "fish" though.
