I noticed I have a tag of "Green Slime" but other people have different ones... Is this based on points or how much you donated or something?

It's based on your "Trophy Points" which you get for doing various things such as writing posts or getting your posts liked by people, etc. So no money, just forum activity

Can I see a list of the requirements or something of the sort?

You can see your trophies by going into profile and clicking on trophy points to the left. But I don't remember seeing any list of all tags.
