Can you check what Twisight is mapped to in the game controls? Is it actually set on Space?
If that's not it... I'm imagining a scenario where this might happen.
If you choose the Big Brothers curse at Candy, the game flags your run as needing Twisight in Dunes. But if you get Big Brothers as a start-of-floor curse... it possibly doesn't do that flag? If that's the case, that's a simple solution for Teddy. Just unlock Twisight at the desert every time.
